Anybody have other numbers for me to try?  Been trying the above one but just get answering service.  Downloaded the forms, need to know what to do with them (scan, email / fax / post / pigeon / horse carriage)?

Sorry! This needs to be updated, the local NCCI no longer does the clearances.
